A distressed man is under some extreme pressure bringing out intense emotions that bring the man closer and closer to his demise.
You really don't know what people are going through because many distressed people seem to appear okay in broad daylight but under the shadows reveals their true inner demons and problems.

The irony of the "Final Thought" is that we don’t really know what his final thought was. The idea is to make the viewer have a subjective thought of what the man is thinking in their own unique way and why he was afraid of life like the quote suggested.
